This session will focus on chronic diseases. Leaders from across the health care continuum—including hospitals, clinics, health plans, and community partners—will discuss the strategies they are using to advance chronic disease prevention, management, and long‑term health. Participants will hear about promising approaches, persistent challenges, and opportunities to strengthen chronic disease care statewide.
